This form explains how RightSite Home Care Inc. collects, uses, and protects your personal and health information in accordance with Ontario’s Personal Health Information Protection Act (PHIPA).

• Safety-related information needed to provide home care

We collect only what is necessary to provide safe and appropriate non-medical home care services.

• Develop and update your Plan of Care
• Schedule and provide home care services
• Communicate with you or your authorized contacts
• Ensure safety and quality of care
• Meet legal and regulatory requirements

4. Sharing of Information

Your information may be shared only with caregivers providing your services, authorized family members or substitute decision-makers, emergency services (in emergencies), or when required by law. We do not sell or use your information for marketing.

5. Storage and Protection

We take reasonable steps to protect your information from loss, theft, or unauthorized access. Records may be stored securely in paper or electronic form.

6. Your Rights Under PHIPA

You have the right to access your personal information, request corrections, withdraw consent (with reasonable notice), and ask questions or make a privacy complaint.
