This Agreement outlines the terms under which RightSite Home Care Inc. (“the Agency”) provides non-medical home care services to the Client in Ontario.
The Agency provides non-medical support services only, which may include personal care (bathing, dressing, grooming), companionship and supervision, meal preparation, light housekeeping, mobility and transfer assistance, and errands and daily living support.
The Agency does not provide medical, nursing, or clinical services.
Services are delivered in accordance with a separate Plan of Care, developed with the Client or Substitute Decision-Maker, and may be reviewed and updated as needs change.
Service schedule, fees, billing and cancellation terms are as set out in the fields below. Invoices are due within the stated number of days; late payment may result in suspension or termination of services. Late cancellations or no-shows may be billed at the scheduled rate.
The Client or family agrees to provide accurate and complete information, maintain a safe and respectful home environment, inform the Agency of any changes in condition or needs, and secure pets during service hours (if applicable).
The Agency complies with Ontario’s Personal Health Information Protection Act (PHIPA); privacy is governed by the separate PHIPA Agreement. By signing, the Client consents to receiving non-medical home care services, Agency caregivers entering the home, and emergency services being contacted when necessary for safety.
Complaints may be directed to the Manager at ComplaintsRightsite@gmail.com. Either party may terminate with the written notice stated below, except in cases of non-payment, safety concerns, or abuse/harassment of staff. The Agency provides services with reasonable care and skill and is not responsible for outcomes beyond the scope of non-medical home care, except where required by law.
This Agreement, together with the Plan of Care and PHIPA Agreement, represents the entire understanding between the parties and is governed by the laws of the Province of Ontario.
